Weekly Practice Log Worksheet
A seven day practice log worksheet with a column for each part of a useful entry, ready to print or copy into a spreadsheet.

This worksheet is the log itself, laid out so that a time only entry is impossible. Every row asks for the focus item, the specific goal, the minutes spent, a quick rating of how it felt, a note about what happened, and a note about what to try next time. The sample rows below mix a music example and a sport example so you can see how each reads; replace them with your own items and add a row for every focus item, not just every day.
Fill it in at the end of each session while the details are fresh. A row does not need to be long. Two or three short phrases per cell is plenty, and the rating column is there so younger students can record something honest in a few seconds. Bring the sheet to the lesson or training session, or share a photo of it, so the teacher or coach can read the week and build the next assignment from it.
